Can different Jotform forms create leads and self check-ins?

Yes. BytePhase uses each Jotform form ID to tell forms apart, so one Jotform integration can carry several forms, each mapped separately. For each form you choose whether a submission becomes a lead or a self check-in, and whether to skip or create a new record when the customer already exists.
